InFRE Volunteer Application Form

Thank you for submitting your application to serve as a member of InFRE’s Board of Standards and Policy Development (BOS) or standing committee. To review the description of the responsibilities of the BOS and standing committees please go to section VIII of this application or go to Volunteer Opportunities.

In our effort to find qualified volunteers we use a comprehensive selection process. This includes collecting background information and requesting comments on open-ended questions. After initial screening of applications, candidates will be interviewed. InFRE may conduct a background check on each applicant under consideration. The extent of this check will depend upon the type of opportunity you are applying for and may include various state and federal searches. If you choose not to provide this information, InFRE cannot process our application. Your submission of the volunteer application form is verification of your acceptance of these terms. All information submitted in the application will remain confidential. However, InFRE may disclose personal information in response to a subpoena or when we believe in good faith that the law requires it or to respond to an emergency situation.

Instructions on completing form

Software Requirements:

To view, complete and print the voluteer form you will need Adobe Acrobat 7.0 Reader (or later) software installed on the computer. The latest version of Adobe Acrobat Reader can be downloaded for free at http://www.adobe.com/products/reader.html

You will be able to fill out the form on a computer and save it to a disk or hard drive for future editing and use. When you are ready to submit the completed form, click the ‘submit’ button on page 5 of the application. You may also attach the completed form and email to Amy Hayes. You may fax the signed completed form and bio/resume to 847-756-7350. Finally, you can mail the signed completed form and bio/resume to InFRE at P. O. Box 524 Barrington, IL 60011-0524.

When you click the submit button on page 5 a pop-up box give you two options, to send with your desktop email like Outlook or an internet email like Yahoo or gmail.

Sending in completed form

Please note by sending your completed application and bio/resume you are agreeing that everything on and attached is correct and accurate to the best of your knowledge. Add your email address to the signature line if emailing the completed form. If faxing or mailing, please sign.

Using desktop email (Outlook, Outlook express, etc)

  1. Select desktop email and click ok. This will open a new email message with the completed application attached and addressed to Amy Hayes.
  2. Attach your bio/resume to email with application.
  3. Send email to Amy Hayes.
  4. Close application form.

Using Internet email (Yahoo, gmail, etc)

You will need to save the completed form.

  1. Select internet email option. This will open the save box. Browse to where you would like to save your completed application. Click save.
  2. Open your internet email application
  3. Start new email message
  4. Attach your saved application form.
  5. Attach your bio/resume to email with application.
  6. Send email to Amy Hayes.

Fax

  1. Print out completed application form
  2. Sign and date application form
  3. Combine with bio/resume
  4. Fax application and bio/resume to 847-756-7350

Mail

  1. Print out completed application form
  2. Sign and date application form
  3. Combine with bio/resume
  4. Mail to InFRE, P. O. Box 524 Barrington, IL 60011-0524
